Some years back I worked as a secretary for the local GP, this was in the 1970′s before doctor’s offices became computorized. The basic form of patient and data management consisted of a simple card index system, whereby a card was filled out with name, address, details etc. The doctor would take card and patient for consultation and write on the card about treatment management. All patients accounts were hand written as well as were receipts. Now, this system worked well enough at the time but, thinking back, it was not cost effective at all especially if a patient’s card got lost.
